On May 11, 2026 — 75 days from now — every healthcare organization with 15 or more employees that receives HHS federal funding must comply with WCAG 2.1 Level AA. That includes every hospital, clinic, and provider participating in Medicare or Medicaid. Non-compliance risks losing that funding entirely.

We analyzed 37,466 healthcare URLs from LLMSE's classification database across four quality dimensions to measure how prepared the industry actually is. The answer: healthcare websites are among the most trusted on the web, and among the least accessible for the patients who need them most.

Healthcare outperforms the web on trust. It matches the web on accessibility. And it underperforms on readability — writing content too complex for the 88% of Americans who lack proficient health literacy.

EEAT: Healthcare Ranks Second Across All Industries

Healthcare sites score 71.0% on EEAT — 23 percentage points above the web average of 47.8%. Only Science (75.0%) scores higher.

Google classifies healthcare as YMYL (Your Money or Your Life), applying the highest content quality scrutiny to pages that "could potentially impact a person's health, financial stability, or safety." Healthcare organizations know this. They invest in author credentials, medical review processes, organizational schema, and the trust signals that EEAT scoring measures.

The Healthcare Industry subcategory leads at 73.8% pass rate (4,475 of 6,066 graded). Dental Health follows at 81.8%, though with a smaller sample (63 of 77). Mental Health trails at 62.5% — sites that prioritize patient empathy over organizational credibility markers.

WordPress Drives Healthcare Trust

WordPress runs 48.6% of all healthcare sites in our dataset (18,196 of 37,466). And WordPress healthcare sites significantly outperform the sector average on EEAT:

WordPress's B-grade concentration tells the story: 62.0% of WordPress healthcare sites score B on EEAT. The platform's plugin ecosystem — Yoast for schema, author bio plugins, contact form builders — systematically produces the trust signals that EEAT rewards. Healthcare organizations using WordPress don't need to think about trust markup; their theme and plugins handle it.

WCAG: The Gap That Matters Most

Healthcare's WCAG pass rate of 54.5% is barely 2 percentage points above the web average. For an industry facing a federal compliance deadline in 75 days, "average" is a crisis.

Mental Health sites lead accessibility at 62.0% — 8 percentage points above the Healthcare Industry subcategory. Mental health services disproportionately serve populations with cognitive and neurological conditions, which may drive greater attention to accessible design. Or it may reflect the younger, more web-native organizations in this space.

The Healthcare Industry subcategory — hospitals, clinics, large health systems — scores worst at 53.7%, with a 28.9% failure rate. These are precisely the organizations the HHS deadline targets.

Readability: Writing for Doctors, Not Patients

Healthcare's readability pass rate of 63.9% falls just below the web average of 64.8%. For an industry whose audience is patients — not practitioners — this is a failure.

The CDC recommends health information be written at an 8th-grade reading level. The NIH and AMA recommend 6th grade. Published studies consistently find healthcare content written at 9.6 to 10.7 grade level — 4 to 5 grades above recommendations.

Our data confirms the pattern. 14.3% of healthcare URLs score F on readability — content so dense it requires graduate-level reading ability. Another 21.8% score D. Combined, over a third of healthcare content is inaccessible to the average American reader before any disability is considered.

Health Education resources — the content specifically created to inform patients — score highest at 76.9%. Healthcare Industry content — hospital and clinic websites — scores lowest at 58.4%. The sites patients visit most often are the hardest to read.

Only 12% of Americans are proficient in health literacy. The gap between how healthcare organizations write and how patients read is not a content strategy problem — it's a public health problem.

SEO: Healthcare Slightly Beats the (Terrible) Average

Healthcare SEO scores 2.8% passing — marginally better than the web-wide 2.0%. Like every other industry, healthcare overwhelmingly fails SEO: 92.4% of graded sites score F.

The 0.8 percentage point advantage likely comes from healthcare organizations' investment in structured data (Organization schema, medical practice schema) and the trust signals that overlap with technical SEO requirements. But 2.8% passing is still 97.2% failing.

The Convergence Problem

Healthcare faces a unique convergence of pressures that no other industry shares:

Regulatory: The May 11, 2026 HHS Section 504 deadline requires WCAG 2.1 Level AA compliance for all HHS-funded entities with 15+ employees. Non-compliance risks suspension of federal funding, including Medicare and Medicaid reimbursements. For most healthcare providers, that's existential.

A site that scores A on EEAT but F on WCAG and Readability is trusted by Google but unusable by the patients who need it.

Key Findings

1. Healthcare has the widest trust-accessibility gap of any industry

Healthcare ranks 2nd in EEAT (71.0%) but 4th in WCAG (54.5%). The 16.5 percentage point gap between trust and accessibility is the largest of any industry we analyzed. Healthcare builds credibility better than almost anyone — and then locks out the patients that credibility is meant to serve.

2. More than 1 in 4 healthcare sites critically fail accessibility

The 27.5% F-rate on WCAG automated checks — covering only 30–40% of Level A criteria — means the real non-compliance rate against Level AA is substantially higher. With 75 days to the HHS deadline, these sites need structural remediation, not overlay band-aids.

3. Healthcare readability fails the patients who need it most

Content written at college level for an audience where 88% lack proficient health literacy. Healthcare Industry sites (hospitals, clinics) score 58.4% on readability — the worst subcategory in their own sector. Health Education resources score 76.9% — proving it's possible to write clearly about complex medical topics.

4. WordPress healthcare sites overperform on trust but match the sector on accessibility

WordPress's 84.9% EEAT pass rate in healthcare is 14 points above the sector average. But WordPress healthcare WCAG (57.5% pass, 24.8% F-rate) is only marginally better than the sector. The WordPress plugin ecosystem excels at generating trust markup. It does less for HTML accessibility — that depends on theme choice, and 24.8% of WordPress healthcare sites chose poorly.

5. Mental Health leads healthcare accessibility

Mental Health sites pass WCAG at 62.0% — 8 points above Healthcare Industry's 53.7% — and score the lowest F-rate on readability (10.1%). This subcategory demonstrates that healthcare can build accessible, readable websites. The rest of the industry should be studying how.

What This Means for the May 2026 Deadline

The HHS Section 504 rule is the first time federal regulation has required all recipients of HHS financial assistance to comply with specific digital accessibility standards. The rule took effect July 8, 2024. The compliance deadline is May 11, 2026. It has not been delayed or modified.

Our data suggests that roughly half of healthcare websites would fail even the automated subset of WCAG 2.1 Level A checks. The HHS requirement is Level AA — a stricter standard. The compliance gap is large, the timeline is short, and the consequences (loss of federal funding) are severe.

Organizations that have invested in trust signals — and our data shows most healthcare organizations have — need to redirect that same energy toward accessibility. The industry knows how to build credible websites. It now has 75 days to make them usable.

Methodology

This analysis cross-referenced LLMSE's Health category (37,466 classified URLs, excluding sanctioned jurisdictions) with quality grade indices as of February 25, 2026. Cross-referencing was performed using Redis sorted set intersections (ZINTERSTORE) between the Health category index and grade indices (seo-{A-F}, eeat-{A-F}, wcag-{A-F}, readability-{A-F}).

Quality grades were generated by LLMSE's analyzers: SEO (80+ on-page checks), EEAT (E-E-A-T content quality signals per Google's Quality Rater Guidelines), WCAG (15 automated WCAG 2.1 Level A checks covering approximately 30–40% of Level A criteria), and Readability (Flesch Reading Ease scoring with web-optimized thresholds).

Industry comparisons used the same intersection methodology across eight major categories, each with 250+ graded domains on each dimension. Not all classified domains have been analyzed on every quality dimension — SEO has the deepest coverage (15,092 graded health URLs), followed by EEAT (9,488), Readability (1,918), and WCAG (1,739).

Limitations: Quality scores reflect the initial HTML response only. WCAG automated checks detect a subset of Level A criteria — the HHS requirement is Level AA, which is a stricter standard. Actual non-compliance rates are certainly higher than our automated scores indicate. Subcategories with fewer than 100 graded domains on a given dimension are marked with an asterisk.
